Perimenopause: What Nobody Told You (But Should Have)
Hot flashes are just the beginning. Perimenopause can start years before your last period and affect everything from sleep and mood to weight and brain function. Here's what's actually happening - and what you can do about it.
You're in your early 40s — maybe even your late 30s. You're sleeping terribly. Your mood is unpredictable in ways that feel foreign to you. Your periods have gotten weird. You're gaining weight around your midsection despite nothing changing in your diet. You feel anxious in a low-grade, hard-to-name way. Brain fog has moved in like an uninvited houseguest.
You go to your doctor. Everything comes back "normal." You're told you're too young for menopause.

What you may be experiencing is perimenopause — the transitional phase leading up to menopause that can begin anywhere from 2 to 12 years before your final period. It is real, it is significant, and it is wildly underdiagnosed and undertreated. And you deserve to understand what's happening in your body.
What Is Perimenopause, Exactly?
Perimenopause literally means "around menopause." It's the phase during which your ovaries gradually produce less estrogen and progesterone, and the hormonal rhythms that have governed your cycle for decades begin to shift.
Menopause itself is defined as 12 consecutive months without a period — typically occurring between ages 45 and 55, with the average around 51. But perimenopause can begin a decade earlier, and for many women, it's the perimenopausal years that are the most symptomatic and disruptive.
The challenge is that hormone levels during perimenopause don't decline in a smooth, predictable line. They fluctuate — sometimes wildly. Estrogen can spike higher than it ever did in your reproductive years before dropping. Progesterone tends to decline first and more steadily. This hormonal turbulence is what drives so many of the symptoms women experience.
The Symptoms Nobody Warned You About
Most women know about hot flashes. Fewer know about the full range of perimenopausal symptoms, which can include:
Sleep disruption — Often one of the earliest and most impactful symptoms. Night sweats are the obvious culprit, but progesterone (which has a calming, sleep-promoting effect) declining before estrogen means many women experience insomnia, light sleep, or early waking long before hot flashes begin.
Mood changes — Anxiety, irritability, low mood, and emotional reactivity that feel out of character. Estrogen plays a significant role in serotonin and dopamine regulation. As levels fluctuate, so can mood — sometimes dramatically.
Brain fog — Difficulty concentrating, word-finding problems, memory lapses. This is one of the most distressing symptoms for many women because it can feel alarming. It is, in most cases, hormonal and temporary — but it deserves to be taken seriously.
Irregular periods — Cycles may become shorter, longer, heavier, lighter, or simply unpredictable. Skipping periods is common. This variability is one of the hallmarks of perimenopause.
Weight changes — Particularly increased fat accumulation around the abdomen. This isn't just about calories; it's about the metabolic effects of shifting estrogen and cortisol, changes in insulin sensitivity, and the loss of muscle mass that accelerates in midlife.
Joint pain and stiffness — Estrogen has anti-inflammatory properties. As levels decline, some women notice new aches, particularly in the hands, knees, and hips.
Heart palpitations — Estrogen influences the cardiovascular system, and fluctuating levels can cause palpitations or a racing heart, particularly at night or around hot flashes.
Vaginal dryness and changes in libido — Declining estrogen affects vaginal tissue and lubrication, and the combination of hormonal shifts, sleep deprivation, and mood changes can significantly affect sexual desire.
Increased sensitivity to stress — The hormonal buffer that progesterone provides begins to thin. Many women find that stress hits harder and recovery takes longer during perimenopause.
Why Standard Lab Work Often Misses It
One of the most frustrating aspects of perimenopause is that standard hormone testing — a single FSH or estradiol level drawn at a routine appointment — often looks "normal." This is because hormone levels during perimenopause fluctuate so dramatically that a single snapshot rarely captures the full picture.
FSH (follicle-stimulating hormone) is often used as a marker, but it can be normal or even low during perimenopause, particularly in the early stages. A single "normal" result does not rule out perimenopausal hormonal shifts.
This is one of the reasons so many women are dismissed or told their symptoms are stress, depression, or anxiety — and sent home with an antidepressant rather than a conversation about hormones.
You know your body. If something has shifted, that matters — regardless of what a single lab value shows.
A Naturopathic Approach to Perimenopause
The naturopathic philosophy is to support the body through this transition as gracefully as possible — addressing the root causes of symptoms, supporting the systems that are under strain, and helping each woman find what works for her individual picture. There is no one-size-fits-all approach.
Here are the foundational areas I work through with perimenopausal clients:
Nourish Your Adrenal Glands
As the ovaries produce less estrogen and progesterone, the adrenal glands take on a larger role in hormone production — particularly producing a precursor hormone called DHEA that can be converted to estrogen and testosterone. If your adrenals are already depleted from chronic stress, this transition is much harder.
Supporting adrenal health means managing stress, prioritizing sleep, eating regular balanced meals, and in some cases using adaptogenic herbs like ashwagandha, rhodiola, or maca to support the stress response.
Stabilize Blood Sugar
Insulin resistance tends to increase during perimenopause, and blood sugar instability amplifies nearly every perimenopausal symptom — hot flashes, mood swings, sleep disruption, weight gain, and brain fog. Eating protein and fat with every meal, reducing refined carbohydrates and sugar, and avoiding long gaps between meals are foundational.
Support Estrogen Metabolism Through the Gut and Liver
The liver and gut play a critical role in metabolizing and clearing estrogen. When this process is impaired — due to poor diet, gut dysbiosis, constipation, or liver burden — estrogen can recirculate in forms that contribute to symptoms. Supporting detoxification pathways through cruciferous vegetables (broccoli, cauliflower, Brussels sprouts, kale), adequate fiber, and liver-supportive foods and herbs is an important piece of the puzzle.
Prioritize Phytoestrogens Thoughtfully
Phytoestrogens are plant compounds that weakly mimic estrogen in the body. Foods like flaxseed, organic soy (tempeh, edamame, miso), and certain legumes contain phytoestrogens that some women find helpful for symptom management. The research is nuanced, and individual response varies — but for many women, incorporating these foods is a gentle and supportive strategy.
Protect Bone and Muscle
Estrogen plays a protective role in bone density, and the perimenopausal years are a critical window for bone health. Resistance training — lifting weights, bodyweight exercises — is one of the most important things a woman can do during this time. It supports bone density, preserves muscle mass, improves insulin sensitivity, and supports mood. Adequate calcium, magnesium, vitamin D3, and vitamin K2 are also essential.
Address Sleep Directly
Sleep deprivation compounds every other perimenopausal symptom. Beyond sleep hygiene basics, specific strategies that can help include magnesium glycinate before bed, keeping the bedroom cool, and addressing night sweats with breathable natural fabrics. Some women find significant relief from targeted herbal support — black cohosh, valerian, and passionflower have evidence for perimenopausal sleep and hot flash relief.
Consider Hormone Testing — the Right Kind
Comprehensive hormone testing — including estradiol, progesterone, testosterone, DHEA-S, cortisol (ideally a diurnal pattern), thyroid panel, and fasting insulin — gives a much more complete picture than a single FSH value. Functional testing through a practitioner who understands hormonal health can be genuinely illuminating.
Hormone replacement therapy (HRT) is also a conversation worth having with a knowledgeable provider. The landscape around HRT has shifted significantly in recent years, and for many women it is a safe and effective option. This is a personal decision that deserves individualized guidance — not a blanket yes or no.
